The spirometer is used by either breathing in or out through the hose. The airflow causes a small white ball to ooze into the tube, indicating the amount of air being passed through the spirometer. The resistance can be adjusted on the small dial on the front of the spirometer.
You can use the spirometer to train both exhalation and inhalation (turn the spirometer upside down).
You can find lots of inspiration to practice with the spirometer on Youtube, we have shared an example on the site.
If you really want to optimize your training, we highly recommend the book Breath Building by Ingrid Holck and Mogens Andresen.
